General instructions:

  1. These are effective from 01-July-2015
  2. Please go through eMudhra Verification Guidelines and Application Procedure for detailed specifications.
  3. Please fill the form in BLOCK LETTERS only.
  4. Inconsistent/incomplete applications are liable to be rejected.
  5. All subscribers are advised to read Certification Practice Statement and Subscriber agreement.
  6. Email ID in the application should be a valid and active email ID belonging to Applicant, in order to issue the certificate.
  7. Mobile Number in the application should be a valid and active Mobile Number belonging to Applicant, in order to issue the certificate.
  8. For Class 3 Digital Certificates "In-person Verification" of the applicant is mandatory.

Document Proof:

  1. All Address proof documents should have the complete address for the purpose of accepting the same as proof.
  2. Document period should not for older than 3 Months for:
    1. Telephone Bill
    2. Water Bill
    3. Gas Connection
    4. Electricity Bill
    5. Bank Statement
  3. Document period should be of current Financial year for:
    1. Service Tax / VAT / Sales Tax Registration Certificate
    2. Property Tax / Corporation / Municipal Corporation Receipt
  4. Bank Account Passbook should be containing the Photograph and signed by the individual with attestation by the concerned Bank official.
  5. Attestation of ID and Address proof documents by any:
    1. Gazetted Officer
    2. Bank Manager
    3. Post Master
  6. Attestation of Organization Existence documents is mandatory by any one of below:
    1. Gazetted Officer
    2. Bank Manager
    3. Post Master
    4. Authorized Signatory, signing the Authorization Letter.
  7. A self-attested copy of ID proof of Attesting Officer is mandatory in all cases (other than foreign).
  8. In case of Foreign Individuals, the attestation is required on Application and the Supporting Documents.
    1. If applicant is in India, or any other country outside the native country, the attestation should be made by the native country embassy.
    2. If applicant is in his/her native country, the attestation should be through apostilization / consularization by the native country. (Public notary is a prerequisite for apostilization / consularization).
